Level 1: Contextual Influences in Diabetes Self-Management Education (SME) Programs

Diabetes self-management education (SME) programs are holistic approaches to support people living with chronic conditions to improve their health status, clinical outcomes, and quality of life. SME interventions are enhanced when coupled with chronic disease self-management support (SMS). SMS comprises ongoing services provided by the program or health care provider to promote sustained improvement to outcomes in the long term. Counseling vulnerable populations is a challenge for many healthcare providers. This presentation will address contextual influences on chronic disease as well as comparing and contrasting motivational interviewing (MI), guided self-determination (GSD) and illness integration support as a means to counsel vulnerable groups. Additionally, this presentation will include case studies that provide practical application for bridging contextual influences or barriers with patient counseling to promote improvements in their health outcomes.

Mosadi  Brown, BA, BASc, MEd, RD, CDE is a Registered Dietitian in private  practice.  Mosadi has an Honors Bachelor of Arts in Kinesiology and an  Honors Bachelor of Applied Science in Food and Nutrition from Western  University.  She went on to complete a Comprehensive Graduate Dietetic  Internship Program from London Health Sciences Centre in London, ON.  After  gaining extensive practice experience in private and public practice Mosadi obtained her Master of  Education in Adult Education with a focus on chronic disease education  and support, in the community.  She holds certifications as a Certified  Diabetes Educator and Certified Craving ChangeTM Instructor.

As a Registered Dietitian – Mosadi’s focus is on educational and behavioural approaches to support sustainable changes for healthy  eating.

Mosadi provides one-on-one counseling services in West Toronto, Halton, and Peel regions.  She is also an avid writer and published author.

When she is not working, Mosadi enjoys spending time with her partner, family and friends, tea, a good book, trying out new cuisines, movies, travel, country drives, and hiking outdoors.
